Request Mediasite Recording

CIS103, ECE241, etc.
Room number that the class is in.
Please include all days and the entire length of class that is to be recorded, not just the class start time. Example: MW 1:30-2:20
Name of the instructor in charge of the class.
Recording Information
Should the Mediasite start recording 5 minutes before you class starts and end 5 minutes after the scheduled end time; or do you want to manually start and stop the recording?
The default layout is to have whatever is showing on the projector as the main area of the recording, with a smaller PIP of the instructor camera.
Please describe the alternate layout, if requested.
